Diagnosis and symptoms.

Spondyloarthoparthy diagnosed June 2008.  Ankle, knee, hip, SI,  low back, elbow, shoulder, and SC joint pain.  Sore and aching muscles, mild fatigue, mental fog, mild to moderate malaise, memory problems.  Food sensitivities.  Sensitivity to prolonged (>2 hours) sun exposure.

-List all meds, supplements (both prescription and OTC) you have been taking and why.

Enbrel (50 mg once per week) for the last 5 weeks on Rheumatologist's orders to attempt to treat Spondyloarthopathy.  Have not noticed any effect yet.  Tramadol (50 mg) as needed for pain.
-When and for how long have you taken prednisone or any other immunosuppressant in any form?
Medrol dose pack in March 2008 for 7 days after shoulder surgery to reduce inflammation.
-When and for how long did you take Vitamin D supplements (including any vitamin-mineral supplements), omega-3 supplements and/or any kind of fish oil?
I have been taking multivitamins with Vitamin D for over 10 years.  I have been supplementing flaxseed oil on and off for 3 years.
-Were you taking Benicar when the sample was drawn?
No
-Were you taking an ARB or or an ACE inhibitor when the sample was drawn?
No
-Test results from bloodwork done 6/27/08
1,25-D = 29 pg/ml
25-D = 37 ng/ml

Hi Alex,

Use of immunosuppressants  will lower 1,25-D (especially prednisone which will lower it by 25-40%).  The Enbrel may have significantly reduced your 1,25-D level.  Your 1,25-D is average at  29pg/ml. The population average is 25-29 pg/ml and it is above-average 1,25-D levels (hypervitaminosis-D) that cause many troublesome symptoms. Since you report many troublesome symptoms, either the lab result is in error, low due to Enbrel or your paracrine (tissue) level of 1,25-D is much higher than the measured serum level. This sometimes occurs when the inflammation is in tissues not well perfused by blood (for example nerves, joints).

You have the option to retest when you are off the Enbrel (which isn't necessary unless Doc insists) or to do a therapeutic probe which is our recommendation.

Your 25-D of  37ng/ml is high and reflects your reported vitamin D supplementation. Because 25-D is immunosuppressive, you need to avoid ALL sources of Vitamin D to get it down to a therapeutic level of 12ng/ml or less. Please see Foods To Avoid and The importance of avoiding vitamin D.

 "The 25-D seems to be the most critical factor as to whether the immune system is able to start working. Any level of 25-D above about 20ng/ml is likely to be acting as an immunosuppressant, with an action very similar to that of corticosteroids." Dr. Trevor Marshall, Ph.D.

Your diagnosis and symptoms indicate Th1 inflammation. Please see Symptoms of Hypervitaminosis-D and you may recognize a few more. You will not get well and your health will continue to deteriorate if you don't treat the underlying bacterial cause of Th1 inflammation with the MP. Please see Is the MP an applicable treatment for my disease?

Most supplements and many medications must be avoided on the MP.
