Good Day to all veterans and moderators.
I'm a new recruit and will be seeing a family doctor/general practicioner for the first time in 20 years. Family doctors are in very short supply here, Ontario, Canada, and in desperation I have been to visit clinics a few times as an attempt to uncover the cause of a disabling pain in my upper back/chest. Mostly it was a waste of time other than a CT scan two years ago showed nodules and lesions in my lungs. I have not been to any doctor since then. Blood work and electrocardio at that time showed "normal" and the doctors simply shrugged that they "didn't know what was the cause".
I eat healthy, no meds, no supplements, spend much time outdoors, excercise regularly and presently I am sick with at least 20 of the classic SARC symptoms. I am the opposite of a hypocondriac but will be armed with data on MP and SARC when I visit with a new doctor on Monday.
My question for today is: What initial tests should I insist being done. Cost of most if not all will be covered by a provincial health care plan here, if not I'll pay. The tests with notes that I am aware of are enclosed below. If anyone has advice otherwise, I will appreciate.

D METABOLITES tests
1,25 dihydroxyvitamin -D
25 hydroxyvitamin - D
NOTE:
-Please perform the following Vitamin D tests: 1,25-D and 25-D.
-Please ensure that collection staff arrange for centrifuging and freezing of the 1,25D sample. (only)
-The sample should be allowed to settle and clot at room temperature for at least 30 minutes (but no more than two hours) and then centrifuged. Do not hold on dry ice prior to centrifuging.
-After centrifuging, freeze at between -2 and -10 C.
-The sample must be transported on dry ice in order to remain frozen until it reaches the testing lab.
ACE or angiotensin-converting enzyme
C Reactive Protein
Triglycerides
Alkaline Phosphatase
SED rate
Complement C3a assay - C3 / C3a Monoclonal Antibody
Soluble Interleukin 2 Receptor (sIL-2R)

Hi Willy,
Welcome to our MP questions board. It's good that you're being proactive about your health troubles and you've been doing some reading.
You've got it right about the tests - the vital ones are the two vitamin D ones. If you can get the others done, well and good. My endocrinologist requires a full panel of kidney function and liver function tests every six months or so (but here in the UK we don't have to pay!). Sarc is assumed if the ACE is very high, but some people with biopsied sarc never have elevated ACE.
A biopsy to confirm sarc is not necessary, although it's the only definitive thing a specialist will want. But if you have sarc symptoms and your vitamin D test analysis indicates Th1 inflammation, then the MP is right for you. We all have a mixture of pathogens, and calling a cluster of symptoms sarcoidosis is just a convenient label.

OK Julia, I am reassured that I am at least starting off in a reasonably good direction regarding benchmark tests to take place in the next week or two. Standard blood work, electrocardio and a CT are planned as well so I may have some comparitive data and hopefully with assistance from moderators will attempt to reference it to my records of two years ago. It may help, I think, to determine the rate of decline in my health and the progress of TH1 related illnesses I've been coping with for the past 24 months. There isn't much question I am worse.
I was informed that a lung specialist will be consulted. I'm interested to speak with the specialist and get their input on a few subjects of interest. I will do the best I can to be objective and inquire about issues applicable to my condition. Most of what is of concern to me has had posts submitted in great technical detail on the MP site and here on CureMyTh1. I suspect the Pulminary Doc will be distinctly out of sync in regards to the leading edge teachings of which I have recently been privledged to be enlightened.
After reading through many posts that seem related to me, my condition and future prospects I feel like I've had a crash course over the past weekend. This is good and I will continue to research, learn and attempt to help others if able.
