I've been following the MP Phase 1 guidelines since Sept 08 and would like the MP Questionnaire to see if I'm ready for Phase 2, but haven't been able to locate it. Would you please direct me to the questionnaire and further instructions? I've got an appointment with my doctor tomorrow and need to be able to provide him with the instructions for what to do next.

Ask Doc to set you up ready for phase 2, just like his/her other MP patients.
PM me for a copy of the questionnaire. I have been working as hard as I can to open up the MP study-site enrollment again, but I can't make any promises at this point as to how, or exactly when, we are going to be able to move forward. Meanwhile you should settle back and continue to 'enjoy' Phase 1 until we can figure out how to support, you, and the others waiting to move to Phase 2.

My doctor doesn't have any other MP patients yet. What does PM mean? Do you want my doctor to email or call you to request the questionnaire? Are you able to fax it to him so we can go over it together during my appointment this afternoon? Here is his contact info just in case:

Anna,
As you know, we had to close membership in our study site in May 2008. I have been working day and night to get it reopened, but we are not quite there yet.
We are still not ready to support new phase 2 members whose physicians are not familiar with the protocol. Since you have not been on phase 1 very long I suggest you stay in phase 1 until you can get the support you will need to move forward.
Please ask Doc to have your 25-hydroxyvitamin-D and 1,25-dihydroxyvitamin-D measured by Quest Diagnostics. Until your 25-D falls into the therapeutic range it is premature to worry about an early transition to phase 2, in any case.
